Physical Therapist - Home Health (Full-Time or PRN) $10,000 Sign-On Bonus

Territory options: Richmond/Chesterfield OR Richmond/Hanover/Caroline

James River Home Health is actively hiring two Physical Therapists to support our growing patient population in the Richmond area. As a Home Health Physical Therapist, you will provide skilled physical therapy services to patients in their place of residence, helping them improve mobility, restore function, and maintain independence while working closely with physicians and the interdisciplinary team to deliver exceptional patient care.

Responsibilities

• Complete patient evaluations and reassessments
• Develop and implement individualized treatment plans
• Provide skilled physical therapy services in the home setting
• Collaborate with physicians and the interdisciplinary team to coordinate care
• Complete OASIS and clinical documentation accurately and timely
• Assess patient progress and adjust treatment plans as needed
• Supervise PTAs as appropriate

Qualifications

• Graduate of an accredited Physical Therapy program
• Current Virginia Physical Therapist license
• Current CPR certification
• Home health experience preferred but not required
• Valid driver's license and reliable transportation
