Hospice on-Call registered nurse (Rn)

Schedule: Monday–Friday | 4:00 PM – 12:00 AM

As an On-Call Hospice RN, you will provide after-hours nursing support, symptom management, patient assessments, and end-of-life care in patients' homes and care facilities. You'll work independently while collaborating with an interdisciplinary team to ensure patients receive exceptional, patient-centered care focused on comfort, dignity, and quality of life.

• What you'll do
• Respond to after-hours patient and family needs from 4 PM to midnight.
• Perform comprehensive patient assessments and evaluate changes in condition.
• Develop, implement, and update individualized plans of care.
• Provide symptom management, medication education, and end-of-life support.
• Educate patients, families, and caregivers on disease progression and care needs.
• Coordinate care with physicians, hospice aides, social workers, chaplains, and other team members.
• Document patient visits and interventions accurately and timely.
• Provide clinical oversight and direction to hospice aides and LVNs/LPNs as needed.

Qualifications

• Current RN license in good standing.
• Current CPR certification.
• Valid driver's license and auto insurance.
• Minimum of 2 years of hospice nursing experience preferred.
• HomeCare HomeBase experience preferred.
• Strong assessment, communication, and critical thinking skills.
