Coordinate holistic care for adults with mental health and medical needs within a team-based Behavioral Health Home model. Responsibilities include assessing client needs, connecting them to vital resources, and advocating for their stability and self-sufficiency.
Requires a high school diploma and one year of relevant adult behavioral health experience as an MHRT-1. MHRT-C certification and an Associate's degree in a related field are preferred.
This is a 40 hour/week, hourly position. Specific schedule to be discussed during interview.
Position based in South Portland, ME and serving Cumberland County.
High school diploma or equivalent with one year of relevant adult behavioral health work experience as an MHRT-1 required. Associate’s Degree in the field of mental health, psychology, or a related field (behavioral health or human services) or a combination of education and experience that qualifies the candidate to receive an MHRT-C certification preferred.
MHRT-C certification or (level A or B with a strategy to receive full certification) Must have experience doing direct service with adults who struggle with mental health. Prefer case management experience with adults who have serious and persistent mental illness and have a history of trauma, substance use, homelessness, poverty, chronic pain and serious health issues. One year of experience required, two preferred. Must be able to successfully pass a criminal background, child protective service check & sex offender check. Must not be on the state or federal suspension and disbarment list. Must be able to exert moderate physical effort with occasional standing, walking, reaching, stooping, bending, kneeling, crouching, frequent typing and lift and carry material weighing up to 25 pounds. Driving may be necessary to attend meetings and trainings and for the occasional need to meet with clients offsite and within the program service area. This may include transportation of clients or driving to agency location to complete work. A good driving record is required.
