• Our therapy department works closely with nursing, physicians, and case management to develop individualized treatment plans that help residents achieve their highest level of function and independence.

• Responsibilities Provide skilled occupational therapy treatments under the direction of a licensed Occupational Therapist Implement individualized treatment plans and physician orders Assist patients in improving activities of daily living (ADLs), self-care skills, upper extremity function, balance, coordination, and overall independence Monitor patient progress and communicate updates to the Occupational Therapist and interdisciplinary care team Complete accurate and timely daily treatment documentation and progress notes Participate in care conferences, discharge planning, and interdisciplinary team meetings Educate patients, families, and caregivers regarding therapy programs and home exercise activities Maintain therapy equipment and treatment areas according to facility standards Support facility goals for patient outcomes, customer service, and quality care Qualifications Current California Certified Occupational Therapy Assistant (COTA) license in good standing Graduate of an accredited Occupational Therapy Assistant program Skilled nursing, post-acute, rehabilitation, or geriatric experience preferred New graduates with strong clinical rotations are encouraged to apply Experience with Net Health, Casamba, PointClickCare, or similar EMR systems is a plus Excellent communication and interpersonal skills Ability to work effectively in a collaborative healthcare environment Passion for helping patients maximize independence and quality of life Ideal Candidate You are someone who: Loves helping patients achieve meaningful functional goals Brings positivity, compassion, and professionalism to every interaction Thrives in a fast-paced rehabilitation environment Values teamwork and clinical excellence Enjoys working with geriatric and post-acute populations Join Our Rehab Team Today!
