• Our facility offers: Private rooms with en-suite bathrooms Spacious common areas designed to promote social engagement A wide range of activities supporting emotional and social well-being Modern therapy equipment and dedicated rehab spaces A collaborative team culture supported by PACS Services, allowing us to focus on resident-centered care Responsibilities Under the direction of a licensed Occupational Therapist, the COTA will: Provide individualized therapy interventions to help residents improve independence and functional abilities Assist with therapeutic activities to enhance mobility and activities of daily living (ADLs) Collaborate with an interdisciplinary team to implement and adjust care plans Accurately document treatments, progress, and outcomes in accordance with regulatory guidelines Deliver care with professionalism, compassion, and attention to detail Qualifications Active COTA certification and California state licensure required Strong commitment to patient-centered care Excellent communication and teamwork skills Experience in a skilled nursing or post-acute setting preferred
