• This position is responsible for: Case Worker monitors assigned caseload of clients participating in short-term life management; established program plan/goals and evaluates client’s progress by conducting routine meetings with client and/or staff.
• Key responsibilities: Interviews applicants based on established guidelines and requirements; conduct needs assessment, obtain pertinent information, and establish program goals for eligible clients. Develops intake and case plans for all assigned clients; instructs clients in completing necessary paperwork including goals and/or budget forms. Meet regularly with clients to discuss and evaluate their progress; prepare accurate and up-to-date records documenting the same; receives incident reports and enforces disciplinary actions for infractions of the house guidelines.

• EXPERIENCE: Two years of college coursework from and accredited college or university in Social Work, Behavioral Science, or a related field, AND Two years experience working in a social or public service environment with experience assisting the public OR Any equivalent combination of training and experience which provides the required knowledge, skills, and abilities. Ability to read, write, and communicate the English and Spanish languages.
