Requires a PharmD or equivalent degree and a valid Registered Pharmacist license in the State of New York. Preferred candidates have completed a PGY1 residency or hold board certification in a pharmacy specialty.
This is a temporary, benefitted opportunity for a pharmacist who wants to take a more focused, patient-centered approach to care. What You’ll Do You’ll serve as a key partner in patient care, helping guide medication decisions and ensure the best possible outcomes. Take ownership of medication management for a core group of patients Review and monitor medication therapies to ensure safety and effectiveness Collaborate with physicians, nurses, and care teams on treatment plans Provide recommendations to optimize therapy, including cost-effective options when appropriate Counsel patients on how to use their medications safely and confidently Document and track medication-related interventions and outcomes What You’ll Bring We’re looking for someone who is thoughtful, detail-oriented, and passionate about patient care. Required Education and Experience License Requirement License to practice as a Registered Pharmacist in the State of New York Education and Experience 1. Doctor of Pharmacy degree; or 2. Master’s degree and the equivalent of one (1) year of full-time clinical pharmacy or pharmacy-related experience; or 3. Bachelor’s degree and the equivalent of three (3) years of full-time clinical pharmacy or pharmacy-related experience. NOTES: 1. Incumbents must be credentialed as a member of the organization Medical Staff (Allied Health Professional) within four months of date of hire as a condition of continued employment. 2. Required degrees must have been granted by an accredited college or university or one recognized by Roswell Park Comprehensive Cancer Center as following acceptable educational practices.
At least 1 of the following: Completed PGY1 Pharmacy Practice Residency Board certified In the process of completion of PGY2 Pharmacy Specialty Residency Program Experience in all 3 of the following:1. 3+ years of work experience in a position of Clinical Pharmacist Specialist (PGY1 Pharmacy Residency accounted for 3 years of clinical experience Experience in of the following: Contributions to scholar activities (within the last 3 years) such as research, publications, and poster presentation. Experience in developing programs contributing to pharmacy practice and patient safety and experience.
