Summary: Our pharmacists are essential members of our health care team and will interact with our patients, providers and all other team members to deliver medications safely and effectively to our patients. In this out-patient setting, our pharmacists prepare prescriptions for a wide variety of patients including hospital discharges, Emergency and Urgent Care patients, community members, Hospice patients and hospital staff. Our pharmacists serve as a readily accessible resource for our patients and other team members. Our pharmacists engage in Medication Therapy Management and Specialty Medication programs routinely.

Functions: Demonstrates the ability to interact in a positive and helpful manner with patients, visitors, physicians, volunteers, and co-workers. Reflects commitment to building a supportive work environment and maintains a positive attitude. Supervises, coordinates, and participates in the preparation, labeling and dispensing of drugs, compounds and other pharmaceutical supplies. Ensures compliance with Federal, State and local laws and regulations pertaining to the dispensing of drugs and narcotics and maintenance of all required records. Ensures safe, appropriate, cost effective medication therapies for patients according to established policies, procedures, and protocols. Monitors drug therapies regimens for contraindications, drug-drug interactions, drug-food interactions, allergies, and appropriateness of drug and dose. Provides clinical consultation and clarification to practitioners. Suggests appropriate, cost-effective therapeutic alternatives to medical staff, as needed. Documents all clinical activities accurately and completely. Oversees and directs pharmacy support staff. Verifies the daily activities of pharmacy technicians. Appropriately uses resources, such as inventory and supplies. Performs other duties as assigned or needed to meet the needs of the department/organization.
