The Care Coordinator facilitates communication between providers to ensure seamless service provision. They are responsible for ensuring individuals receive appropriate care tailored to their specific behavioral health needs.
A Bachelor's degree in social work, human services, or a related field is required. Experience in behavioral health care coordination and an active Iowa BSW license are preferred.

About the Job Seasons is a comprehensive behavioral health center offering a broad range of psychiatric and behavioral health services to communities in the Northwest Iowa region. We’re currently seeking a full-time Care Coordinator. The Care Coordinator will help coordinate service provision for providers, facilitating communication between providers, and ensuring individuals receive the appropriate care for their individual needs.
A Bachelor’s degree in social work, human services, or a related field is required Experience providing care coordination services for individuals with behavioral health concerns is preferred but not required An active Iowa Bachelor’s of Social Work (BSW) license is preferred but not required Questions About the Job?
