RN/LPN Osceola County (Four Corners/Poinciana) for SOC/Visits.

Matrix Home Care is seeking experienced and dependable Registered Nurses and Licensed Practical Nurses to provide skilled nursing services in patients’ homes throughout the Four Corners and Poinciana areas of Osceola County. This position is ideal for nurses with home health experience who are comfortable completing Start of Care assessments and ongoing skilled nursing visits. Candidates must be compassionate, organized, clinically confident, and committed to delivering safe, high-quality care.

Responsibilities

Complete Start of Care assessments and assigned home health visits Perform comprehensive patient assessments Provide skilled nursing care according to physician orders and the established plan of care Complete routine visits, follow-up visits, resumptions of care, recertifications, and discharge visits, as assigned Provide wound care, medication management, disease-management education, and other skilled nursing services Educate patients and caregivers regarding treatments, medications, safety, and the plan of care Monitor patient progress and report changes in condition to the physician and clinical team Coordinate care with physicians, therapists, caregivers, and other interdisciplinary team members Complete accurate and timely clinical documentation Follow agency policies and all applicable state, federal, Medicare, and home health requirements

Requirements

Current and unrestricted Florida Registered Nurse or Licensed Practical Nurse license Previous home health experience preferred Start of Care and OASIS documentation experience strongly preferred for Registered Nurse applicants Strong clinical assessment, communication, documentation, and time-management skills Current CPR or Basic Life Support certification Valid driver’s license Reliable transportation and current automobile insurance Ability to travel throughout Four Corners, Poinciana, Kissimmee, and surrounding areas Successful completion of an Agency for Health Care Administration Level 2 background screening Ability to work independently in patients’ homes
