The RN Case Manager performs patient assessments and collaborates with a care team to implement and manage plans of care. Responsibilities include supervising LPNs and HHAs, providing patient education, and maintaining accurate clinical documentation.
Candidates must have a current RN license for the assigned state, CPR certification, and a valid driver's license with liability insurance. At least one year of clinical experience is preferred, though candidates with less may be eligible via approval or an intern program.
