The Care Coordinator conducts assessments for elderly and frail clients to develop and maintain comprehensive care plans. They provide ongoing case management, facilitate referrals, and ensure accurate documentation of services provided.
A Bachelor's degree in a related field and IDOA certification upon hire are required. Candidates should possess strong organizational skills and the ability to travel independently within assigned communities.
