The Care Coordinator will conduct timely assessments for elderly and frail clients in community settings, develop comprehensive care plans, and provide ongoing case management and referrals. Responsibilities also include ensuring accurate documentation, collaborating with internal teams, and traveling within assigned communities to support high-quality client care.
A Bachelor's Degree in Arts/Sciences is the minimum education requirement, with 1-3 years of relevant experience preferred, especially in Care Coordination or Case Management within the aging network. Candidates must possess strong organizational, communication, and problem-solving skills, along with a valid driver's license and mandatory IDOA Certification upon hire.
