Conducts participant assessments and presents findings at care plan conferences to establish foundations for care. Provides ongoing education and support to participants and families regarding aging, dementia, and end-of-life issues while managing external referrals.
Requires a Master's Degree in Social Work and at least two years of experience in skilled rehabilitation. Proficiency in Microsoft Office applications and experience working within multidisciplinary teams is preferred.
