Collaborates with clinical teams to manage patient risk factors and facilitate comprehensive discharge planning across inpatient and outpatient settings. Provides psychosocial interventions and mental health education to support patients and families in navigating the healthcare system.
Requires a Master's degree in Social Work, Mental Health, Psychology, or Marriage and Family Therapy. Candidates must have two years of direct clinical experience, which can be substituted by a Master's level internship in the relevant population.
Master’s degree from an accredited school of Social Work, Mental Health, Psychology or Marriage and Family Therapy is required.
Licensure/Certification Handle with Care (HWC) Certification required for Behavioral Health Unit.
Experience Two (2) years of direct clinical experience with an emphasis on the population to be served in the assigned area. Successful completion of Master’s level internship within the population to be served may substitute the two (2) years of experience.
