Requires a Bachelor's degree in Social Work, Counseling, Psychology, Human Services, or a related field. Preference is given to candidates with knowledge of community behavioral health, crisis services, and experience working with children and families.

The Patient Family Support Specialist in the Children's Crisis Intervention Center coordinates safe, timely, and person-centered transitions of care for patients presenting with acute behavioral health needs. This role partners closely with child psychiatry, nursing, clinical social work, child life, protective services, and community stakeholders to develop, implement, and document discharge plans that align with identified needs, clinical recommendations, legal/regulatory requirements, and the patients and families preferences. The Patient Family Support Specialist also follows up on access to services post-discharge, and works on projects/task forces as assigned by supervisor or manager to improve quality of care and interdisciplinary collaboration.
