Provide personalized end-of-life care to patients in an inpatient hospice setting, assisting with daily activities and comfort measures. Collaborate with the interdisciplinary clinical team to implement care plans and maintain accurate electronic documentation.
Requires a high school diploma or GED and a valid driver's license with insurance. STNA or CNA certification and at least one year of direct patient care experience are preferred.
