Responsible for coordinating medical needs and reviewing documentation for individuals with intellectual and developmental disabilities in residential settings. Supports the RN by managing appointments, medication logs, and accompanying individuals to medical visits.
Requires a high school diploma or GED and at least one year of experience as a Direct Support Professional with Medication Certification. Must possess a valid NYS Driver's License, a reliable vehicle, and the ability to lift up to 50 lbs.
Sign-on bonuses are paid as follows for new hires to the organization in this job title: $500 for per diem status (less than 30 hours per week) $750 for part-time status (30 - 37 hours per week) $1,000 for full-time status (37.5 - 40 hours per week) Sign-on bonuses are divided into four equal payments paid out at three months, six months, nine months, and one year of employment.
Full-time, 37.5 hours per week. Primarily Monday - Friday weekdays, with flexibility across all shifts as required by the needs of the agency and the individuals we support. Please note that travel is required (mileage is reimbursed). This position will be assigned to sites in Edwardsville, Canton, and Norfolk, though locations may be adjusted in the future.
This position shall be responsible for assisting with the medical needs of people with intellectual and developmental disabilities in Individual Residential Alternatives (“IRAs”).
1. Responsible for routine review of required medical documentation and logs in the IRAs for accuracy and completion. a. Visit all assigned sites at least once per week. Review the communication logs, appointment books, and minor incident tracking. Report all issues to the RN. b. Review BM charts, MARs, and controlled counts for issues and communicate to the RN. c. Complete monthly and weekly medication cupboard checks as directed by the RN. Ensure medication storage is appropriate and expired medications are disposed of and replaced.
4. Responsible for additional duties as assigned by the Director of OPWDD Services or the RN.
POSITION REQUIREMENTS 1. High School diploma or GED is required. 2. A minimum of one year of experience as a Direct Support Professional with Medication Certification is required. 3. Ability to pass all OPWDD-required background checks. 4. A NYS Driver’s License with two years of driving experience is required. 5. Acceptable motor vehicle abstract to meet Agency insurability standards. 6. Reliable personal vehicle for business use. 7. Effective verbal and written communication skills and knowledge in electronic health record usage. 8. Skilled in organizing, planning, exercising initiative, judgment, problem-solving, and maintaining effective relationships with providers, staff, individuals supported, and the public required. 9. Medium work; ability to lift and carry up to 50 lbs., lift and support individuals with and without help. 10. Ability to operate personal and Agency vehicles. 11. Ability to sit, walk, and stand for extended periods of time. 12. Some exposure to infectious diseases, hygiene issues, food, and other odors. 13. Ability to operate mechanical devices used by individuals. 14. Ability to tolerate extensive exposure to computer monitor/screen. 15. Ability to tolerate extensive use of computer keyboard.
