Essential
Duties and Responsibilities
- Patient Monitoring and Care
- Monitor and document patient status during Phase II recovery, including vital signs, pain levels, mobility, and overall condition.
- Observe for and promptly report changes in patient condition to the RN, anesthesia provider, or surgeon.
- Assist patients with ambulation, dressing, toileting, and preparation for discharge.
- Administer medications and treatments as directed and within the LPN scope of practice.
- Reinforce post-operative care plans and physician instructions.
- Patient and Family Education
- Provide clear and effective discharge education to patients and caregivers regarding:
- Pain management
- Medication administration
- Wound care and dressing changes
- Activity restrictions and mobility precautions
- Signs and symptoms requiring medical attention
- Follow-up appointments and therapy expectations
- Verify understanding through teach-back methods.
- Answer questions and address concerns to promote a safe recovery at home.
- Care Coordination
- Collaborate with RNs, surgeons, anesthesia providers, physical therapy, and other team members to facilitate patient discharge.
- Ensure discharge criteria are met and required documentation is completed.
- Communicate patient needs and concerns effectively to the healthcare team.
- Documentation and Compliance
- Accurately document assessments, interventions, patient education, and discharge instructions in the electronic medical record.
- Maintain compliance with regulatory standards, infection prevention practices, and organizational policies.
- Participate in quality improvement initiatives and patient safety programs.
Qualifications
- Required
- Current Licensed Practical Nurse (LPN) license in good standing.
- Current BLS certification.
- Strong communication and patient education skills.
- Ability to work efficiently in a fast-paced surgical environment.
- Excellent organizational and time-management abilities.
