The LPN provides one-on-one skilled nursing care and family education to patients within their place of residence. Responsibilities include following established plans of care, maintaining timely documentation, and collaborating with an interdisciplinary team.
Candidates must be graduates of an accredited LPN program with a current state license and CPR certification. A valid driver's license, automobile insurance, and reliable transportation are also required.

The LPN administers skilled nursing care to patients in their place of residence as prescribed and in accordance with the established plan of care. The LPN is also responsible for timely documentation and collaboration with the interdisciplinary team.
Graduate of an accredited Licensed Practical Nurse educational program Current Licensed Practical Nurse (LPN) license within the state of practice Current CPR certification Excellent verbal, written, organizational, collaboration, multi-tasking and time-management skills Strong initiative, follow through, process orientation, attention-to-detail Reliable transportation, valid driver’s license and current automobile insurance
