The Care Manager RN independently utilizes the nursing process to provide safe and efficient care for a group of cardiology patients. This includes conducting shared and independent visits while collaborating with a multidisciplinary team to execute care plans.
Candidates must be a graduate of an accredited nursing school with a valid Washington Registered Nurse License and at least 2 years of professional nursing experience. A Bachelor's Degree in Nursing and experience in ambulatory care are preferred.
Under the general direction of the Supervisor/Manager of Clinic Operations with clinical oversight from the Director of Clinical Operations, RN: The Team Based Care (TBC) Care Manager RN independently uses the nursing process to care for a group of patients safely, therapeutically, and efficiently. This role conducts shared patient visits with providers as well as independent RN visits using standardized procedures/protocols and evidence-based workflows. The TBC RN collaborates with other team members to coordinate and execute the plan of care. Demonstrates initiative in the delivery of patient care in a Team Based Care Model.

Posted are the minimum and the maximum wage rates on the wage range for this position. The successful candidate's placement on the wage range for this position will be determined based upon relevant job experience and other applicable factors. These amounts are the base pay range; additional compensation may be available for this role, such as shift differentials, standby/on-call, overtime, premiums, extra shift incentives, or bonus opportunities.
