The Ambulatory Care Nurse is responsible for providing nursing care management services to assigned populations, utilizing a collaborative approach to coordinate the patient's plan of care to maximize community living. The nurse acts as a patient/family advocate and communication link between the physician, patient, family, healthcare team, and community resources.
The role requires a Registered Nurse license and a minimum of a Nursing Diploma or Associate's Degree in Nursing, with no prior experience being required for this level I position. The nurse must support organizational mission and adhere to professional nursing standards and practice models.
