The Physical Therapist will conduct comprehensive evaluations in patients' homes, developing individualized, evidence-based treatment plans to improve mobility and functional independence. Responsibilities also include educating patients/caregivers, monitoring progress, documenting care, and coordinating with the interdisciplinary team.
Candidates must be a Licensed Physical Therapist (PT) in Michigan with CPR certification and a minimum of 1 year of clinical experience, preferably in home health within the last 5 years. A valid driver's license, reliable transportation, and strong clinical and communication skills are mandatory.
