The Nurse Practitioner will perform Face-to-Face encounters essential for hospice eligibility certification and recertification, gathering clinical information and conducting assessments. They will collaborate with physicians and interdisciplinary teams to ensure quality patient care and compliance with regulations.
Candidates must have a Master's or higher degree in Nursing, a current registered nurse license, and national nurse practitioner certification. Strong understanding of hospice care principles and excellent communication skills are also required.
