The Cardiac Care Advanced Practice Provider offers services to nursing center residents in collaboration with the AMP physician, providing consultation and education to nursing staff regarding cardiovascular care specific services. This role involves being a member of the Cardiac Care Consultation team, focusing on the diagnosis, treatment, and prevention of complications related to cardiovascular disease for patients and families.
Candidates must be a Nurse Practitioner with a Master’s Degree in Nursing or a Physician Assistant with NCCPA certification and state licensure, both requiring a minimum of two years of experience working in Cardiology. A preference is noted for a minimum of two years of experience working with the adult or geriatric population.

We are seeking a Full Time (40 hours per week) Cardiac Care provider for a float position between our centers located in Fairmont, Morgantown, and Grafton, WV. This position can be filled by a nurse practitioner or physician assistant. Cardiology experience is required.
The Cardiac Care Advanced Practice Provider reports directly to the Area Clinical Practice Manager with a dotted line to the Director/Sr. Director of Clinical Practice. The Cardiac Care Advanced Practice Provider collaborates on-site with the Director of Nursing for center-specific goals and priorities and with the Medical Director and/or primary care APPs for patient-specific medical care.

Nurse Practitioners and Physician Assistants are Advanced Practice Providers with master’s degree preparation in an expanded role who provides direct care to individuals, families, and/or groups in a variety of patient care settings. In collaboration with the physician and other staff, (s)he is responsible for the diagnosing, prescribing treatment, and providing follow-up for patients during phases of wellness and illness. (S)he initiates referrals for implementing the patient’s total care plan involving other health disciplines within and outside the hospital. (S)he coordinates the care a patient may need in the community and oversees the case management of the patient and is accountable for keeping up to date with changes. Nurse Practitioners and Physician Assistants place a marked emphasis on patient teaching and education, and promote competent, compassionate nursing care for patients and families.
The Cardiac Care Advanced Practice Provider is a member of the Cardiac Care Consultation team providing cardiac care consultation to medical teams, patients and families, as well as nursing, social work, and rehabilitation therapies.
The Cardiac Care APP serves as a role model to facility nursing, therapy, and medical practitioners regarding the care of individuals who are formally diagnosed and at risk for cardiovascular disorders.
The Cardiac Care APP focuses on the care of the individual patient and family for the diagnosis and treatment of cardiovascular disease, prevention of complications of cardiovascular disease, and coordination of cardiovascular care both at the center and during transitions of care. The practice of the Cardiac Care APP is directed towards continuity of care from hospitalization to discharge and home. Attention is on competent clinical management, informed decision-making, safe environment and education regarding the spectrum of their cardiac disease. Work is performed with general independence and decision-making responsibility in accordance with cardiac care clinical guidelines, scope of practice and the patient’s attending physician.

Restrictions apply based on collective bargaining agreements, applicable state law and factors such as pay classification, job grade, location, and length of service. Eligibility for full-time benefits requires a minimum of 30 hours per week. Certain benefits, including Health plans, are not available to part-time employees working between 16 and 29 hours per week.
