Part-time Nurse Practitioner or Physician Assistant - In Home Wellness and Chronic Care - Central / West LA

• Homebase Medical is expanding rapidly into the Central / West Los Angeles area.
• We are currently searching for experienced part-time Advanced Practice Clinicians (Physician Assistant or Nurse Practitioner) to join our expanding house call practice.
• Key Duties: Conduct Annual Wellness Visits focused on patient education Deliver Chronic Care Management to improve long-term outcomes Perform comprehensive in-home visits including: Medication review & chronic illness management Medical history updates & physical exams Labs, diagnostic screenings, and post-hospital follow-ups Primary care management & palliative care services We seek Rebels who are curious about AI and its power to transform how we operate and serve our members.

Qualifications

Active Nurse Practitioner or Physician Assistant license in the State of California Board certification (Nurse Practitioner or Physician Assistant) DEA licensure Prior experience preferred (but passion and commitment are just as important What's in it for you?
