Key duties include conducting Annual Wellness Visits focused on patient education and delivering Chronic Care Management to improve long-term outcomes for homebound older adults. Clinicians will also perform comprehensive in-home visits covering medication review, physical exams, labs, and primary care management.
Candidates must possess an active Nurse Practitioner or Physician Assistant license and Board certification in the State of California, along with DEA licensure. Prior experience is preferred, but the employer values passion and commitment highly.
