The Social Worker is responsible for delivering clinical and supportive services to senior residents in supportive housing, focusing on promoting housing stability, wellness, independence, and quality of life through person-centered, trauma-informed care. Key duties include conducting psychosocial assessments, providing crisis intervention, coordinating care, and maintaining accurate documentation in compliance with agency and funder requirements.
Candidates must possess a Master's degree in Social Work from a CSWE-accredited school or a CASAC, along with current, active NYS licensure (e.g., LMSW, LCSW) and at least two years of specialized social work practice experience. Preferred qualifications include 2-3 years of experience working with formerly homeless individuals or those in supportive housing, along with strong knowledge of aging-related conditions and experience with trauma-informed models.

The Social Worker delivers person-centered, trauma-informed, and strengths-based services, including psychosocial assessments, crisis intervention, care coordination, and short-term counseling. The role supports residents who may be experiencing chronic medical conditions, behavioral health challenges, cognitive decline, social isolation, and histories of homelessness or housing instability.
