The Advanced Provider will deliver high-quality primary and palliative care to homebound patients in the Atlanta County, GA area by performing physical examinations, obtaining medical histories, and developing comprehensive care plans. Responsibilities also include diagnosing conditions, prescribing necessary interventions, and fostering trust through effective consultation with patients, families, and caregivers.
Candidates must be Board Certified with a valid Nurse Practitioner or Physician Assistant certification in Georgia, possessing strong clinical skills for accurate assessment and diagnosis. A minimum of 1-2 years of primary care experience with geriatric patients is required, and hospice experience is highly preferred.

We are looking for an experienced Advanced Provider (Nurse Practitioner or Physician Assistant) to provide primary care to homebound and palliative care patients in the South Atlanta, Vinings area of Georgia area on a full-time basis. You will apply medical techniques and principles under the supervision of a physician, and you will care for a diverse set of patients with chronic diseases.
• $105,000- $145,000+ (depending on experience and other incentives) (full Time)
