The Host Home Provider will offer support to an individual with a developmental disability within their residence to meet their needs in adherence to the Iowa Medicaid waiver program. This involves assisting with daily living activities, preparing meals, providing transportation, helping with medications, and implementing support plan programs.
Applicants must be at least 18 years old, possess a valid driver's license with car insurance, and successfully pass all required background checks. Preference is given to candidates with at least one year of experience working with individuals who have developmental disabilities.

Hands of Heartland is looking for compassionate and caring individuals to share their home with a person with developmental disabilities as a Host Home Provider. For this position, there should be no children in the home.
Provide supports to a person with a developmental disability in your home to ensure their wants and needs are met in compliance with the Iowa Medicaid waiver program. This can include assisting with activities of daily living, meal planning and preparation, providing transportation, assisting with medications, and implementing programs in accordance with the persons Individual Support Plan.
