The Case Manager is responsible for the provision and facilitation of comprehensive care management, services, and quality outcomes, focusing primarily on patient transition to the appropriate level of care while ensuring clinical appropriateness and managing Length of Stay (LOS). This role involves directly supporting hospitalized patients needing comprehensive case management, promoting effective utilization of health services, and collaborating with the team and patient/caregiver to manage care and transitions.
A minimum of 12 months of nursing experience is required for this full-time position, which involves working four 10-hour shifts on weekend days (Friday, Saturday, Sunday, and Monday) in the Emergency Department. The role requires developing comprehensive care plans based on physical, mental, and psychosocial needs.
