This role involves conducting comprehensive psychosocial assessments, developing integrated discharge and treatment plans, and coordinating care with internal and external providers for patients adjusting to illness and lifestyle changes. The social worker also provides essential emotional support, advocacy, and crisis intervention services to patients and families navigating complex medical and social challenges.
Candidates must possess a Master's degree in Social Work (MSW) and hold a State Licensure (LICSW), along with proficiency in English communication and computer skills. Preferred qualifications include acute care hospital social work experience and obtaining CPI certification within one month of starting.
