The Renal Licensed Master Social Worker will assess patients' psychosocial status, identify treatment barriers, and collaborate with the multidisciplinary team to develop and implement individualized care plans. Responsibilities also include educating patients and families on End-Stage Renal Disease issues, facilitating transplant referrals, managing admissions, and completing required documentation.
Candidates must possess a Master's Degree in Social Work from a CSWE-accredited school and hold current state licensure or appropriate certification to practice. Required experience includes working with renal patients and demonstrated skills in communication, customer service, quality improvement, relationship development, and handling insurance-related patient issues.
As a Renal Social Worker (LMSW), you will play a key role in supporting our patients' well-being. Your responsibilities will include: Assessing patients' psychosocial status and identifying barriers to successful treatment as part of a comprehensive assessment. Collaborating with the multidisciplinary team to develop and implement individualized care plans that improve patient outcomes and quality of life. Providing patient and family education on psychosocial issues related to End-Stage Renal Disease (ESRD) and available support services. Working with the interdisciplinary team to educate patients on all treatment options and facilitate transplant referrals. Managing the patient admission process to ensure a smooth transition and adherence to the care plan. Completing all required documentation to support the billing process.
EDUCATION Master's Degree in Social Work with a concentration in clinical practice from a school of Social Work accredited by the Council on Social Work Education (CSWE). Current stale licensure or appropriate certification/registration by the slate if required in order to practice social work or be called a Social Worker. EXPERIENCE AND REQUIRED SKILLS Experience with renal patients required Demonstrated leadership competencies and skills for the position, including: o Excellent communication skills- verbal and written, Excellent customer service, Practices continuous quality improvement, Good relationship development skills, Sound decision making Experience with helping patients deal with insurance related issues.
