Deliver evidence-based, patient-centered care to medically complex patients through comprehensive assessment and individualized treatment plans. Coordinate with interdisciplinary teams and families to manage care and facilitate safe discharge planning.
Requires a Bachelor's Degree in Physical Therapy and a valid State of California Physical Therapist License. Current Basic Life Support (BLS) certification from the American Heart Association is also mandatory.
As the Supervisor Social Worker, Special Services, you will help the manager plan, organize and direct the Social Work Special Services department. In this role, you will supervise a team of 10 social workers and oversee daily patient care operations. You have a unique opportunity to make a meaningful difference in the lives of people living with HIV and AIDS across Fresno, Kings, Madera, Merced and Tulare counties. You will help create a welcoming, nonjudgmental environment where patients receive confidential, comprehensive medical care, case management, free rapid HIV testing, risk‑reduction counseling, and assistance accessing needed medications. You are ready to guide the clinic through assignments, scheduling, evaluations, and patient flow. And to inspire team building, communication, and conflict management skills. Since this clinic is grant-supported, budget support and meeting financial and operational goals are priorities.
