Our mission is to advocate, support, and provide services to people with developmental and other disabilities. Our services emphasize choice and full community engagement. The Direct Support Professional provides self-care training and assistance in daily living to individuals with developmental disabilities attending Day Habilitation Program.

SUMMARY

Assist individuals with developmental disabilities with skill acquisition, retention, or improvement in self-help, socialization, and adaptive skills. Assists in the development and attainment of life goals. Provides support and services to individuals to attain independence, productivity, and integration.

• ESSENTIAL DUTIES AND RESPONSIBILITIES include the following.
• Other duties may be assigned: Provides individualized care as outlined in the individual’s Life Plan/ IPOP.
• Assists individuals in the primary activities of daily living (toileting, feeding, cooking, and cleaning) as outlined in the individual’s Life Plan / IPOP to promote independence.
• Maintains files and produces service notes and documentation on assigned caseload.
• Documents and submits reports to the program as outlined in the assigned department.

Requirements

EDUCATION and/or EXPERIENCE High School diploma or GED. Valid NYS driver’s license with a driving record that is in accordance with the requirements of our Liability/Automotive Insurance carrier.
