The Physical Therapist will lead home care services for a dedicated caseload, which includes performing assessments and initiating rehabilitative procedures. They will also coordinate with physicians, acting as the link between the hospital and the patient's recovery at home.
Candidates must possess a current Massachusetts Physical Therapist License and have a minimum of two years of experience in Physical Therapy, with at least one year specifically in Home Care or Public Health. Proficiency in OASIS and current BLS certification are also required.
