This dual-role position dedicates about 50% of time to case management, which involves developing client plans and connecting families with community resources, and 50% to client medical coordination. Responsibilities include coordinating medical appointments, supporting medication management in collaboration with nursing staff, and maintaining accurate case documentation.
Candidates must possess a bachelor’s degree in social work or a related field and be at least 21 years old, along with possessing a valid driver's license and auto insurance. Required qualifications also include being med-aid trained and familiar with electronic Medication Administration Records, alongside passing required background checks.

This position serves in a dual role, with approximately 50% of time dedicated to case management duties and 50% focused on client medical coordination and support. As part of the LSSSD team, the case manager supports and strengthens individuals and families by developing client-centered case plans, providing ongoing assessment, and assisting clients in accessing community resources to meet their needs.
include maintaining accurate case documentation; building collaborative relationships with community partners and service providers; coordinating medical, dental, and vision appointments; ensuring annual exams are completed; supporting medication management processes in collaboration with nursing staff and pharmacy providers; obtaining medical records and treatment consents; reviewing Medication Administration Records (MARs) for accuracy; and transporting clients to appointments as needed.
This position participates in the on-call rotation system.
