The Social Worker coordinates, facilitates, and executes social work functions by collaborating with the Interdisciplinary Care Transitions team to address patient psychosocial needs throughout the care continuum. Essential functions include coordinating psychosocial activities for comprehensive discharge planning, conducting biopsychosocial assessments, and serving as a patient advocate.
Candidates must have experience coordinating psychosocial activities, utilizing critical thinking for discharge planning, and maintaining knowledge of community resources and legal issues affecting patients. A Master of Social Work is preferred, and a Social Work License as required by the state is mandatory, along with three years of experience in a healthcare setting.
