This role manages the development, implementation, and evaluation of care management service lines, focusing on enhancing member management quality through continuity of care and cost-effectiveness across inpatient and outpatient settings. Responsibilities include partnering with internal/external advisors and departments to establish safeguards and outcomes, coordinating preventative care, and collaborating on quality and utilization improvement activities.
The role requires a Registered Nurse (RN) licensure in the state of practice, with a Bachelor's Degree in Nursing (BSN) and Certified Case Manager (CCM) certification being preferred. Essential functions involve collaboration with physicians and staff on resource utilization, performing ongoing medical necessity reviews, and managing discharge planning and utilization management communications.
