The Physical Therapist evaluates and treats patients using physical therapy principles, establishing goals and developing individualized treatment plans based on the patient's diagnosis and age. Responsibilities also include collaborating with the interdisciplinary team, providing patient education, recommending treatment modifications to the physician, and maintaining required documentation.
Candidates must be license-eligible or licensed in Physical Therapy in Illinois, with a Bachelor's degree required, and a Master's or Doctorate degree preferred. Desirable qualifications include 1+ year of outpatient experience and familiarity with EMR/EPIC and Microsoft Office software.
30PM, rotating weekends. New Grads Welcome Evaluates and treats patients using the principles and practices of physical therapy and in accordance with the patient’s age, developmental level, and diagnosis. Collaborates with the interdisciplinary team. License-eligible therapists would perform duties under the supervision of a licensed therapist in accordance with the Illinois Physical Therapy Practice Act. Provide "One on One" treatment to provide each patient with the highest level of care.
