• Provide skilled LPN care in long-term resident settings, including health assessments, medication administration, management of chronic conditions, wound care, and timely response to changes in status.
• Deliver restorative and ADL support, mobilization assistance, and monitoring of safety, hydration, nutrition, and skin integrity.
• Compile accurate documentation, communicate effectively with RNs, physicians, families, and interdisciplinary teammates, and participate in care rounds to refine resident-centered plans.
• Support staff development by mentoring CNAs and collaborating on individualized care plans that reflect residents’ goals, preferences, and cultural considerations.
• Ensure compliance with clinical standards, privacy regulations, infection control protocols, and quality measures, while preserving residents’ dignity and rights.
