As a Long-Term Care LPN, you’ll provide hands-on nursing care, monitor residents’ conditions, administer medications as delegated, perform assessments, and contribute to comprehensive care planning in collaboration with physicians, RNs, CNAs, social workers, and dietary staff. You’ll assist with activities of daily living, ensure safety and infection control, manage documentation in electronic health records, and engage families with clear, compassionate communication. You’ll participate in the development and modification of care plans, identify changes in condition, and implement evidence-based practices to optimize comfort, nutrition, mobility, and quality of life.
