The Speech Therapist will develop and adjust individualized plans of care, guide patients in therapeutic activities to improve functioning, and observe, record, and report patient progress to the physician and staff. Responsibilities also include improving swallowing, cognition, speech, and communication through activities and home exercise programs while minimizing patient risk.
Candidates must possess a current Speech Therapist license in the state of employment and have a minimum of one year of experience as an ST in an acute care setting, with home care experience being preferred. A valid driver's license and auto insurance are mandatory, along with the capability to meet all physical demands of the role.

HOW YOU'LL WORK: You'll provide home-based skilled speech therapy care for patients as directed by the attending physician. You will educate patients and their family members and ensures the safety of the patient. MAJOR AREAS OF RESPONSIBILITY: Plan of Care: Develop an individualized ST plan of care and makes adjustments as needs change. Guide the patient in his/her use of therapeutic, creative and self-care activities for the purpose of improving functioning. Observe, record, and report patient reactions, changes, and progress to attending physician and any appropriate agency staff. Patient Outcomes: Use activities and home exercise programs to improve swallowing, breathing, cognition, speech and communication, and/or any other speech therapy related function. Minimize Patient Risk: Help decrease re-hospitalizations by teaching the patient on preventative measures and making good decision. Family Educator/Advocate: Counsel patient and family on the disease/injury and how to manage in all phases of speech therapy. Counsel and instruct patient and family on the disease/injury/disability and how to manage in all phases of speech therapy.
Speech Therapist with a current license in the state of employment. Minimum one-year experience as a ST in an acute care setting. Home care experience preferred.
