The Occupational Therapist will evaluate patients to determine rehabilitation needs, develop individualized plans of care, and implement/adjust treatment plans to ensure quality outcomes. Responsibilities also include educating patients/caregivers, assisting with necessary equipment, and collaborating with interdisciplinary teams.
Candidates must be graduates of an accredited Occupational Therapy program and hold a current, unrestricted Occupational Therapist license in the state of practice. A minimum of one year of clinical care experience, reliable transportation, and the ability to travel up to approximately 50% within the coverage area are also required.

That’s why we offer: A collaborative, supportive clinical environment The opportunity to make a meaningful difference in patients’ lives Tuition reimbursement for full-time staff and free continuing education opportunities for all employees Career advancement and leadership opportunities Comprehensive benefits for eligible employees including medical, dental, and vision coverage 401(k) with employer match for eligible employees Generous paid time off and paid holidays for eligible employees Family and pet bereavement leave Pet insurance What You’ll Do As an Occupational Therapist with Elara Caring, you will: Evaluate patients to determine rehabilitation needs and develop individualized plans of care Perform initial evaluations and ongoing reassessments according to OASIS and PPS regulatory guidelines Implement and adjust treatment plans to ensure quality outcomes and continuity of care Communicate patient progress, significant changes, or concerns to the physician and care team Educate patients and caregivers on activities of daily living (ADLs), adaptive equipment, and home safety Assist patients and families in obtaining necessary equipment, including custom splint fabrication when appropriate Provide instruction and support for home therapy programs Collaborate with interdisciplinary teams to ensure the highest quality patient care Supervise and provide clinical guidance to Certified Occupational Therapy Assistants (COTA), Home Health Aides (HHA), and other clinical staff Ensure documentation and care delivery meet professional standards and regulatory requirements This role involves working in a physically active and fast-paced environment, including patient transfers, lifting, and travel between patient homes. What You’ll Need Graduate of an accredited Occupational Therapy program Current, unrestricted Occupational Therapist license in the state of practice Minimum 1 year of experience in a clinical care setting Ability and willingness to travel within the assigned coverage area (approximately 50%) Valid driver’s license, reliable transportation, and current auto insurance Ability to lift, push, pull, and support up to 50 pounds, including patient transfers and equipment movement Reporting Structure This position reports to the Branch Director, Clinical Manager, Clinical Supervisor, or designee.
