The Registered Nurse Home Health is responsible for assessing the physical, psychosocial, and environmental needs of assigned home health patients and assuming primary responsibility for developing, implementing, and evaluating their plan of treatment. This role involves making initial and follow-up evaluations during visits and communicating significant findings or changes in patient health condition to the Clinical Supervisor and physician.
Candidates must possess a current, unrestricted state RN license and have at least one year of experience in a clinical care setting. Applicants must also have a dependable vehicle, valid driver's license, current auto insurance, and be able and willing to travel up to 50% within the branch coverage area.
