Requirements summary
Candidates must hold a current, unrestricted state RN license and have at least one year of clinical nursing experience. A degree in nursing and experience with HomeCare HomeBase or home health/hospice care are preferred.

Supportive, collaborative interdisciplinary team environment What You’ll Do As a Home Health RN, you will serve as the clinical leader for your assigned patients, coordinating and delivering skilled nursing care in the home setting. Key responsibilities include:
Conducting comprehensive nursing assessments and developing individualized Plans of Care
Managing a caseload of patients from admission through discharge
Performing start-of-care visits, routine visits, recertifications, and discharges
Evaluating physical, psychosocial, environmental, and functional patient needs
Collaborating with physicians, caregivers, and interdisciplinary team members to ensure continuity of care
Monitoring patient progress and adjusting care plans as needed
Educating patients and families on disease management, medications, and self-care strategies
Identifying early changes in condition and intervening to prevent avoidable hospitalizations
Documenting all clinical findings, interventions, and outcomes accurately and timely
