The Admission Nurse promotes the company's philosophy and mission while maintaining patient and staff privacy according to HIPAA. Responsibilities include assessing hospice patients' physical, psychosocial, and environmental needs and participating in the implementation of the plan of care.
Applicants must be graduates of an accredited nursing program with a current, unrestricted state RN license and at least one year of clinical care experience. Candidates must also have a dependable vehicle, valid driver's license, insurance, and be willing to travel up to 50% within the coverage area.

Home Health Admissions Registered Nurse (RN) Chicopee Service Area/ Full Time At Elara Caring, we don’t just open cases—we set the clinical standard for everything that follows. The admission visit is where skilled home health either succeeds or struggles downstream. That’s why this role is reserved for experienced nurses who understand that strong admissions are built on clinical precision, sharp assessment skills, and disciplined documentation from the very first visit. We are seeking a high-performing Skilled Home Health RN who is confident in the field, strong in clinical judgment, and understands how to build an accurate, defensible foundation of care in the home setting. This is not a training role. This is a role for nurses who have already mastered the rhythm, complexity, and responsibility of home health admissions. What This Role Really Is This is a field-based, high-skill clinical admission role focused on: First-visit comprehensive assessments in the home Establishing clinically sound, compliant plans of care Identifying risks and care needs that drive downstream outcomes Ensuring documentation reflects the true clinical picture—not just checkboxes Why Experienced Home Health Nurses Choose This Role Focused on high-impact admission work, not long-term caseload management Competitive compensation: $85,000–$98,000 annually Strong clinical infrastructure and support without micromanagement Tuition reimbursement and ongoing continuing education Full benefits package (medical, dental, vision) 401(k) with employer match Paid time off, holidays, and family & pet bereavement benefits Opportunity to stay clinically active in home health without being tied to a full case load burden What You Will Do As an Admission Nurse, you will: Complete comprehensive Skilled Home Health admissions in the home setting Perform advanced clinical assessments across physical, psychosocial, and environmental domains Translate patient presentation into a clear, accurate initial plan of care Identify clinical risks, safety concerns, and gaps in care at the point of entry Ensure all admission documentation is complete, compliant, and clinically defensible Communicate findings and significant changes promptly to physicians and care teams (within 24 hours) Make sound clinical decisions that shape downstream care quality and outcomes Maintain accuracy and compliance aligned with internal standards and regulatory expectations What We’re Looking For This role is designed for experienced SHH clinicians who have: Active, unrestricted RN license Minimum 1+ year of Skilled Home Health experience (required) Strong, independent field nursing practice in admissions or high-acuity home visits Working knowledge of HomeCare HomeBase (HCHB) preferred Solid understanding of home health compliance, documentation expectations, and visit structure Confidence operating independently in the field and managing full admission workflows Strong assessment skills with the ability to quickly synthesize clinical information Requirements Associate or Bachelor’s Degree in Nursing preferred Valid driver’s license, insurance, and reliable transportation required Ability and willingness to travel up to 50% within the coverage area Ability to sit, stand, bend, lift, and move intermittently; able to lift 50–100 lbs Equal Employment Opportunity: We are proud to be an equal opportunity workplace and comply with state and federal affirmative action requirements.
