The Registered Nurse Home Health will assume primary responsibility for a caseload, including assessing, planning, coordinating, implementing, and evaluating the plan of treatment for home health care patients. This role involves communicating significant findings and revising the plan of care in response to identified patient issues to the Clinical Supervisor and physician.
Applicants must possess a current, unrestricted state RN license and have at least one year of experience in a clinical care setting as a nurse. Preferred qualifications include an Associate's or Bachelor's Degree in Nursing, previous Home Health or Hospice Experience, and experience with HomeCare HomeBase.

Responsibilities include: Completing skilled nursing assessments and developing individualized plans of care Performing admissions, routine visits, recertifications, and discharge visits Evaluating patients' physical, psychosocial, environmental, and educational needs Coordinating care with physicians, caregivers, and interdisciplinary team members Monitoring patient progress and identifying changes in condition Updating and revising care plans to ensure optimal outcomes Educating patients and families on disease management, medications, and self-care techniques Maintaining accurate and timely clinical documentation Helping prevent avoidable hospitalizations through proactive intervention and patient education Delivering compassionate, patient-centered care while promoting safety and independence Qualifications Required Current, unrestricted Massachusetts Registered Nurse (RN) license Minimum of one (1) year of nursing experience in a clinical care setting Home Care experience is HIGHLY preferred, as this is PRN and having knowledge of how home care operates will help to make you successful Ability to commit to at least one full day (8 hours) per week Valid driver's license, reliable transportation, and current auto insurance Willingness to travel throughout the assigned territory Physical Requirements - Sit, stand, bend, lift and move intermittently and be able to lift 50-100 lbs.
