Physical Therapists are responsible for providing comprehensive initial and ongoing patient assessments, including OASIS documentation, and establishing/revising treatment plans in consultation with the physician. They must also assist in developing interdisciplinary care plans, conduct diagnostic/prognostic tests, and educate patients and families on the physical therapy program.
Candidates must be graduates of an AMA/APTA-approved Physical Therapy school and must be licensed or registered by the State, requiring at least one year of institutional or home health experience. Applicants must also pass a criminal background check and hold a current CPR (BLS) certification.

This position is paid at a competitive salary with the opportunity to earn extra money by picking up visits at our per diem rate Mileage reimbursement for any company travel Medical/Dental/Vision Insurance/Supplemental insurance (begins immediately upon hire) Company 401K with 4% company matching
Provide initial and ongoing comprehensive assessments of the patient's needs, including Outcome and Assessment Information Set (OASIS) assessments Establish a treatment plan in consultation with the physician and revises it with the approval of the physician as necessary Assist in development and implementation of the interdisciplinary patient care plan as it pertains to Physical Therapy Assist the physician evaluating patients by applying diagnostic and prognostic muscle, nerve, joint, and functional ability tests Teach, supervise, and counsel the family and patient in the physical therapy program Write clinical/progress notes on the date of the visit and incorporates same in the patient's record within 72 hours.
The ability to drive and ability to access patients' homes which may not be routinely wheelchair accessible are required Hearing, eyesight, and physical dexterity must be sufficient to perform a physical assessment of the patient's condition and to perform patient care On occasion, may be required to bend, stoop, reach and move patient weight up to 250 pounds; lift and/or carry up to 30 pounds
Must be a graduate of a Physical Therapy school approved by the Council of Medical Education of the A.M.A. in collaboration with the American Physical Therapy Association Must be licensed or registered by the State Must have at least one year of institutional/home health experience Must have criminal background check Must have a current CPR (BLS) certification
