The Physical Therapy Assistant provides patient services delegated by a Registered Physical Therapist and assists in the development of rehabilitative plans. They are also responsible for maintaining clinical documentation and progress notes within the electronic health record system.
Candidates must hold a degree from an approved Physical Therapy Assistant program and possess a current state license. Additionally, a current CPR (BLS) certification and the ability to perform physical assessments and patient care in home settings are required.

How we support you professionally: This position is paid at a competitive per diem visit rate Company 401K with 4% company matching
Under the direction of a Registered Physical Therapist (PT), provide patient services which have been delegated by the PT Under the supervision of the PT, assist in evaluation and development of a rehabilitative plan of care and in periodic re-evaluation as required Participate in the preparation of clinical and daily progress notes Assume other duties deemed appropriate and necessary by the PT and as designated by the Registered Nurse Case Management and the Nursing Supervisor
The ability to drive and ability to access patients' homes which may not be routinely wheelchair accessible are required Hearing, eyesight, and physical dexterity must be sufficient to perform a physical assessment of the patient's condition and to perform patient care On occasion, may be required to bend, stoop, reach and move patient weight up to 250 pounds; lift and/or carry up to 30 pounds
Must be a graduate of an approved school giving a course of not less than two years for Physical Therapy Assistants Must be licensed by the State as a Physical Therapy Assistant Must have a criminal background check Must have a current CPR (BLS) certification
