The Case Manager delivers care coordination services to clients with a history of substance abuse, serving as part of the interdisciplinary treatment team to coordinate services, facilitate support groups, and advocate for client needs.
Candidates must possess a CASAC-T, CASAC, or higher certification, along with at least one year of experience working with clients with substance use disorders and adolescents with co-occurring mental health issues. Strong organizational, networking, and collaborative skills are essential for success in this role.

Position: Case Manager
The Case Manager is responsible for the delivery of care coordination services to a population with history of substance abuse. The Case Manager serves as a member of the interdisciplinary treatment team.

Assistant Program Director in collaboration with Program Director
This is a full-time or part-time non-exempt position.
Sun-Mon: 1pm-9pm
Tuesday-Thursday: 9am-5pm
This job operates in a professional office environment and outside in the field. This role routinely uses standard office equipment such as computers, phones, photocopiers, filing cabinets and fax machines. Must be able to travel among assigned sites and locations as needed.
